Veterinarians: Pets can help with cancer research

HOUSTON (AP) — Leading Texas veterinarians are mobilizing to enlist pets in the testing of experimental cancer therapies, a potential benefit to not just dogs and cats but people.

The veterinarians recently set up a registry they hope will connect pet owners and cancer researchers and show that diseased pets — dogs in particular — are better predictors of the efficacy of new cancer drugs and devices in people than mice, oncologists’ favorite test subject historically.

“Dogs may be man’s best friend in more ways than one,” says Dr. Theresa Fossum, a Texas AM professor of veterinary surgery and founder of the Texas Veterinary Cancer Registry. “Because they suffer from cancers that are nearly identical to those in humans, but quicker to run their course, they can speed up and make more reliable the process of determining whether a therapy will work.”

Veterinarians are just starting to get the ear of cancer researchers, who don’t reflexively think of naturally occurring disease models that go home with their owner, Fossum said. The Cancer Prevention and Research Institute of Texas recently rejected a grant application because “housing the dogs would be so expensive,” says Fossum. The application will be resubmitted to specify that the treated pets would remain with their owners.

Texas veterinary oncologists are hoping the registry can help change attitudes by providing a database of dogs and cats diagnosed with cancer that could be candidates for clinical research. The registry identified its first patient in November, an 8-year-old Great Pyrenees with bone cancer.

Instead of the standard treatment, amputation, Rowdy got an experimental procedure: radiation injected into 22 tiny holes drilled directly into the bone cancer. Two months later, Rowdy’s owner reports he is running around symptom-free, though Fossum stresses the six-month check-up will be the big test.

The procedure’s success in a dog trial would bode well for people with the disease — particularly children. Osteosarcoma, Rowdy’s cancer, is the sixth most common form of childhood cancer. One in 3 diagnosed with the disease die from it.

The idea of using animals’ naturally developing cancers as models for human disease goes back a decade but has never taken off.

“It’s a great concept, but the problem has always been the lack of infrastructures pairing researchers and patients,” said Dr. Peggy Tinkey, chair of veterinary medicine and surgery at the University of Texas M.D. Anderson Cancer Center.

That’s where the new cancer registry comes in. Owners of dogs and cats diagnosed with the disease are being asked to register their pet at http://texasvetcancerregistry.com/, already up and running though the official launch isn’t scheduled until this spring. Registry staff will contact the pet’s vet for more information, then look for potential research matches.

There should be no shortage of candidates. There are 77.5 million owned dogs in the United States and a fourth will develop cancer — including those in the bone, breast, pancreas, liver, prostate, lung and skin. Veterinarians report that owners increasingly want to treat them, at around $5,000, but the options can be limited.

The technology used in Rowdy’s case was pioneered at a Houston company, Valco Instruments, that makes very small, precise instruments used in laboratories. After his dog was diagnosed with bone cancer, the company’s president developed a drill that can open holes the size of human hairs to deliver radioactive isotopes that pinpoint the tumor and don’t damage surrounding tissue.

“It sounded perfect,” said Rowdy’s owner, Kate Cordts, a librarian in San Antonio. “Rowdy’s such an active, happy-go-lucky dog — I just didn’t have the heart for amputation.”

For all their benefits, pets won’t ever replace lab mice as cancer test models. For one thing, mice are perfect specimens for engineering genetic impairments and studying precisely targeted genes or pathways suspected to be involved in a disease. For another, they’re better for establishing initial safety, necessary before experimenting in pets.

Still, Fossum thinks pets can play an important role. She notes that one reason it costs $1.2 billion, probably more, to get a new drug on the market is that most fail in clinical trials. Mice simply aren’t good disease predictors, she says, not like dogs and cats, which live with people, have intact immune systems and probably develop cancer for the same reasons.

The pet cancer registry is just the beginning. Fossum has plans, once she gets grant money, to launch pet registries for heart and kidney disease too.

Immunisations and anal pharmacists

This month, my daughter reaches an important milestone: it’s her last round of immunizations until school age. It will be a relief for me to see her chubby little thighs bandage-free until she’s marching off to school with an overloaded Schultüte.

Luisa and her counterparts in North America and the UK are lucky to count going to the doctor as a fact of life. When a baby is born in Germany, he gets presented with a Babypass. Like my Mutterpass, the little book is a transportable medical file that mothers can take from doctor to doctor with her medical history. It also tracks baby’s development.

All of the check-ups, or Untersuchungen, are abbreviated to U1, U2, U3, etc. Luisa’s appointments are labelled on the front of her Kinderpass with what dates they should fall between, going all the way to April 2016. Each Untersuchung has a page for the doctor to fill out. It also leaves me with a handy little guide to how much Luisa has grown over the last year (it’s a lot!).

Most of the exams so far have been simple physicals. Making sure Luisa is growing properly and all her little parts with it. At the U3, there is an ultrasound to look for hip dysplasia — something that I find Germans to be disproportionately concerned with. As there is some hip joint issues in Luisa’s family medical history, she received an ultrasound at her U2, then again three weeks later, then with a specialist, and then again at the U3. The conclusion: “Her hips are just fine, we just like to be very careful when it comes to hip dysplasia,” said our doctor.

According to the International Hip Dysplasia Institute, hips that require treatment only occur in two to three children per 1,000.

What I do like about Germany’s scheduled medical care for babies is that they do immunizations a little later than in the English-speaking world. While Luisa’s friends in the US, the UK, and Canada all had their first round of shots at two months, Luisa didn’t get her first Impfungen until she was nearly four months. The first round is done in conjunction with the U4 check up, when Luisa is between two and four months old. Since her appointment was booked closer to the end of the fourth, that was simply when she got her first round.

As for the immunizations themselves, they are very much the same as what children in North America and the UK are given. Right now, Luisa has fighting power against tetanus, diphtheria, pertussis or whooping cough, polio, pneumococcus and hepatitis B. On the advice of my paediatrician, I skipped the Rotavirus immunisation and many German parents also leave out the Hepatitis B. After her last round, Luisa will also be armed against measles, mumps, and rubella.

Overall, there isn’t much difference between the care Luisa would get here versus there. But what is different in Germany is the at-home care and what Mamas keep in their at-home medical kit.

I’ve written before on the various uses of breast milk to cure these things, but that does come to an end and now those ailments lead me to the medicine cabinet.

For stuffed noses, we have saline solution. It’s hated by our little one and therefore seldom used. As Germany is the birthplace of homoeopathy, I can find all sorts of natural remedies. My favourite is called Osanit and they’re little pearls I use for teething pain. It’s main medicinal ingredient is chamomile. Whenever those gums start causing problems, babes are dosed with a few little pearls that they can roll around in their mouth and it seems to work. Life goes on. American mom Laurie has a similar product in her cabinet called Dentinox-Gel N, which also contains chamomile but in a gel format.

For those fevers, we use paracetamol, but its application is what makes our stash “very German”. The favoured method of dosing your child by the Mamas is Zäpfchen, or suppositories.

I wasn’t sure what to make the first time I realised what my doctor had prescribed after Luisa’s first round of immunizations in case of fever. But she was hot and miserable and so it happened and it was awful for everyone, but it did make her feel better. The next day I went to the pharmacy and asked for liquid paracetamol to give her instead.

The pharmacist was confused by my request. “But with the suppositories, you know she’s getting the right amount. You don’t have to worry about getting her to swallow it and once it’s done, it’s done,” she said, very pragmatically.

With the next fever, I took out a spoon and tried to get Luisa to swallow her medicine. It didn’t work. Her mouth clamped shut, she shook her head and sticky, orange-flavoured syrup got all over the floor. The practical German in me took over and we went back to the Zäpfchen. I’m now a convert.

I’m not the only one. British mum Tori told me she thinks they’re brilliant. “I would have never used them if I were raising Max [in England], but my husband, who is a doctor, was the one who stocked up the medicine shelf and at first I wasn’t so sure, but now they’re all I use.”

But others aren’t convinced.

When Laurie’s son has a fever, she uses liquid ibuprofen, which her pharmacist told her not to use until he was six months old. Before that, she had infant’s Advil and Triminic sent from the US. She also keeps American-bought Neosporin on hand. She also wishes that children’s acetaminophen were more available in the Fatherland.

English mum Susannah won’t touch the Zäpfchen. “Sticking things up a baby’s bum is not an idea I’d ever considered until I became a parent here. I would have no idea how to go about it, and would worry I was hurting her.”

Instead, she stocked up on the beloved English cure-all Calpol on her last trip home. “It’s poured liberally down English children’s throats from a very young age, whereas Germany seems less into plying babies with drugs,” she explained. “By extension, the German equivalents seemed less trustworthy in my mind: Irrational but true.”

Significant weight loss over a few months without any apparent cause, i.e. unexplained weight loss, can be one of the main warning signs that you might be suffering from cancer. This happens most often with cancers of the pancreas, stomach, esophagus, or lung.

Any oral ulcer that refuses to heal: If you have a mouth ulcer or white plaque in the mouth which has not healed for more than 3 weeks, then you must consult a doctor. If you use any form of tobacco (smoked or smokeless), stop that. This may be associated with constant irritation due to mal-aligned teeth; you must see a dentist and have the problem sorted as soon as possible as constant irritation to the buccal mucosa can also lead to problems.

Long standing cough with or without expectoration or Hemoptysis or hoarse voice: There are many medical conditions that can cause ‘chesty’ symptoms like coughing and breathlessness (for example, infections and inflammations), but in some cases these symptoms may be a sign of lung cancer. If you have a cough or feel breathless for more than two weeks you should see your doctor. You should also tell your doctor if you have any blood in your sputum (phlegm) when you cough.

Changes in the skin: One can witness changes in his/her appearance, boundary or colour in a pre-existing mole or appearance of new lesions. Skin cancer may be developing in some. Especially in people who often chew tobacco, smoke or drink alcohol, their skin sores might not heal.

White patches inside the mouth or white spots on the tongue: It is also known as lenkoplakia, which may progress to oral cavity cancer later. Leukoplakia is a pre-cancerous area that is caused by frequent irritation. If it is not treated, leukoplakia can become oral cancer.

Unusual bleeding or discharge: It can happen in early or advanced cancer. Blood in the sputum (phlegm) may be a sign of lung cancer. Blood in the stool could be a sign of colon or rectal cancer. Cancer of the cervix or the endometrium (lining of the uterus) can cause abnormal vaginal bleeding. Blood in the urine may be a sign of bladder or kidney cancer. A bloody discharge from the nipple may be a sign of breast cancer.

Thickening or lump in the breast or other parts of the body: These symptoms can be caused by many other reasons, other than cancer, and a doctor is the best person to identify the reason after necessary investigations. Instead of waiting for symptoms to surface, it would be better to opt for periodic health checkups.

Itching, crusting or bleeding: These are less common signs but should not be ignored. It is important to see your doctor if you have any unusual marks on the skin that last more than a few weeks, or an existing mole which shows any of the above signs. If necessary they will arrange for you to see a doctor who specialises in skin conditions (a dermatologist) or a surgeon.
